To place an order, please follow these two simple steps: 

1)  Calculate the total cost of the quantity of assessments you require, and type both the quantity and total dollar amount on this email: order@intentionquest.com, then hit send.

2)  Go to www.paypal.com and click on "Send Money", then enter the following email address: michael@intentionquest.com.  Enter the total dollar amount, then follow the instructions to complete your purchase via credit card.  Your payment will be processed and you will receive an email from "PayPal" indicating that your payment was accepted (please print for your records).  You will also recieve a confirming email from IntentionQuest.com.

You will recieve your AFFED Assessment(s) via email; delivered to the email address from which the order was placed (within 24 hours).  This email will include an authorization code, which will be valid for the entire quantity of Assessments ordered.  In cases where multiple instruments have been ordered by one individual/email address, yet are to be distributed to multiple respondents, the individual in recipt of the original email should forward the email to the other users, instructing them to include their name and the authorizatoin code on the assessment.  Once complete, the assessment should be returned to: report@intentionquest.com. (this is indicated on the assessment).

Upon reciept of the completed assessment, we will process the instruments and return the Behavior Analysis Report(s) to the email address from which the assessment was recieved (within 24 hours).  If you have any questions, please contact support@intentionquest.com.
